Descrição do Episódio: Neste episódio, exploramos os resultados de um ensaio clínico randomizado de referência, publicado no Journal of the American College of Surgeons (JACS), que avaliou o uso de tela profilática onlay para reforço do fechamento e prevenção da deiscência da fáscia (FD) em pacientes de alto risco submetidos à laparotomia mediana de emergência.

Apesar dos avanços nas técnicas de fechamento, a FD continua sendo uma complicação devastadora — especialmente em cenários emergenciais e contaminados. Este estudo, conduzido no Hospital das Clínicas da FMUSP, demonstrou que o reforço com tela onlay reduziu a taxa de FD para 0%, em comparação com 13,5% nos fechamentos padrão com sutura. Embora o grupo com tela tenha apresentado maior incidência de complicações menores no sítio cirúrgico (SSOs), mais de 90% foram resolvidas com conduta conservadora ou manejo ambulatorial.
