🧠 Episode Description: In this episode of Master General Surgery and Trauma – Abdominal Wall Series, we dive into a critical topic in neurotrauma: what is the optimal transfusion strategy for patients with severe traumatic brain injury (TBI)? Based on Dr. Estevão Bassi’s lecture and recent clinical evidence, we explore the physiological rationale behind red blood cell transfusion, associated risks, the liberal vs. restrictive strategies debate, and findings from key trials such as HEMOTION, TRAIN, and SAHaRA.
🔍 We cover: • Cerebral hypoxia in TBI and its prognostic impact • When transfusion helps—and when it may harm • Diverging evidence from randomized controlled trials • Identifying patients who benefit most from liberal transfusion • How to individualize decision-making at the bedside
💡 Take-Home Messages: • Cerebral hypoxia is frequent in severe TBI and worsens outcomes. • Transfusion may improve brain oxygenation in select patients—but carries risks. • Transfusion strategy should be tailored to the clinical context and phase of injury. • Recent evidence leans toward a liberal strategy in selected high-risk patients, though not conclusively. • No ideal tool exists yet to predict which patients will respond to transfusion.
