A year ago, the Prime Minister stood in front of abuse survivors and apologised for generations of harm and neglect under successive governments and promised that they would try harder to ensure it never happened again - while setting up some of the exact same mechanisms and some all new ones, that are shown to lead to the sort of abuse these people suffered.
