This Episode of The Senior Podcast Show focuses on Discharge Planning which can often be a confusing and stressful time for those who have had a complicated Hospital or Rehabilitation stay in a Skilled Nursing Facility. Host, Kori Lehr, will be joined by special guest, Jonathan Gasso, Owner of PARC Home Care & Hospice. 

A discharge plan is an interdisciplinary process that determines what type of care a person will be expected to need once they discharge after a Hospital or Rehabilitation stay. There can be many people involved in the process, including Doctors, Nurses, and Social Workers, all with the ultimate goal of ensuring the patient's transition to home is as safe and smooth as possible. Generally a discharge plan will include Evaluations of the patient's current status and abilities, Medication Reconciliation, Determining the level of care the patient will need, Referrals to Home Health or other care facility and any important follow up appointments.
