The team have been recognised locally at UHNMs Night Full of Stars winning clinical team of the year as well as nationally winning the Patient Safety Pilot Project of the Year at the HSJ Patient Safety Awards.

In this episode I am joined by Jules and Annie from the Acute Care at Home team (@_VWNorth). They talk us through their virtual ward criteria, how they help people remain at home or to be discharged from hospital sooner. They talk through some of the challenges and also the amazing feedback showing how vital their service is in preventing unnecessary hospital admission but providing top quality care in a patients own home.

Ian Tyrrell (ACP) and Dr Alice Holt (EM Consultant) are joined by RCEM credentialed ACP Erica Smith (@Erica_J_Smith) to talk about how to assess an older person who presents to an Emergency Department. They talk history taking, investigations and how to discharge someone safely. This episode is the last in the current series.  

Ian Tyrrell (ACP in Frailty) and Dr Alice Holt talk about frailty. They talk about frailty scoring, the geriatric giants and comprehensive geriatric assessment and if it is possible to complete in emergency departments.

Inspired by a piece from the The Book About Getting Older by Dr Lucy Pollock, Dr Alice Holt, Dr Sarah Kelt, Dr Eleanor Cross and Ian Tyrrell explore how to approach having difficult conversations in regards to Respect documents and resuscitation. They explore how prepared they felt when first approaching these conversations and how they become easier after time. They also discuss the emotional burden that these conversations can have and the importance of self-care and good teamwork.
