I loved this conversation with Justin.
In this episode of Paul Meets, I sit down with clinic owner and CrossFit lover Justin Vaidyan (https://www.arisephysicaltherapy.com) from New York, who has gone from zero to almost one million in revenue in a cash focused practice that is almost fully out of network. He started his business during Covid, used his house down payment to sign the lease on an old dry cleaner, and turned it into a modern performance clinic that now runs with multiple therapists and staff.
What I really wanted to pull out of Justin is the shift from great clinician to real business owner. The pricing mistakes, the awkward early marketing, the fear around hiring, and the identity crisis that happens when you stop being “the physio” and start leading a team.
Inside this episode you will hear Justin talk about:
- How he went from losing his job in Covid to starting a clinic and having a baby at the same time
- The decision to spend his house deposit on a tiny ex dry cleaning space and turn it into a premium performance clinic
- The scrappy early marketing that got him from zero to three hundred thousand by joining gyms, running screenings, and forcing himself into the community
- The reality of undercharging at one hundred and fifty a visit, dropping to one hundred and twenty five, and then climbing to two hundred and fifty and beyond
- Why hiring a therapist as his first full time role was a mistake and how he fixed it with a proper operations and admin set up

In this episode of Coffee with Goughy, I talk honestly about a stage of growth most clinic owners hit but rarely say out loud. The moment when success starts creating pressure instead of freedom. When everyone needs you. When the business quietly starts owning you.
I share:
• Why growth often feels worse before it feels better
• The identity shift I had to make as a clinic owner
• Why doing more nearly kept me stuck
• And what actually has to change if freedom is the real goal
No hype. No theory. Just a real conversation for clinic owners who know something isn’t working the way it should.

This isn’t a training.
It’s not a webinar.
And it’s definitely not three tips to fix your marketing.
In this episode of Coffee with Goughy, I go straight to the real reason so many clinic owners feel stuck, overwhelmed, or quietly frustrated, even when the business looks “fine” from the outside.
I talk about leadership.
Standards.
Insecurity.
Delegation.
Pricing guilt.
And the hard truth most owners don’t want to admit: if a problem has been hanging around long enough, it probably isn’t a staff issue. It’s a leadership one.
This is a raw, unfiltered conversation about what actually limits growth in a clinic, why businesses flatline at certain revenue levels, and how your confidence, decisions, and tolerance for discomfort determine how far your business can really go.
No tactics.
No fluff.
Just an honest conversation over coffee about what it takes to build a clinic that works without breaking you.

In this episode, I’m bringing you something special. I just spent two full days in my Orlando office with clinic owners who flew in from all over the world for our Strategic Annual Planning event, and what you’re about to hear is raw, real footage from inside that room. The energy, the breakthroughs, the uncomfortable questions, the big decisions — it is all here.
I walk you through how I challenge business owners to think not just about next year, but about who they must become and what their clinic will need to look like in 2030. You’ll hear me talk about momentum, mistakes, creativity, environment, freedom, and why most clinic owners get stuck in “no mans land” around five staff. I get into the importance of stepping out of your daily environment to stimulate new thinking, why ambitious people need space, and how real growth only happens when you stop building a business for money and start building it for freedom.
If you’re ready to think differently about your life, your clinic, and your future — turn the volume up and enjoy this behind the scenes session. Let me help you start planning and preparing to build a more profitable, systemized, and future ready physical therapy business.

In this episode of Paul on Fire, I go all in on a truth most business owners still aren’t ready to face yet the future of business is agents. AI agents. And if you are not adapting right now, you are already falling behind.
I break down the biggest misconception I see every day. That growth comes from hiring more people. It doesn’t. Growth comes from improving labor efficiency, controlling payroll, and letting technology do what it’s already proven it can do better, faster, and cheaper. I walk you through real examples from my own business, how replacing a 45 to 50k salary with a 15k AI agent instantly improves your labor efficiency ratio and why that single shift can change the entire profitability of your clinic.
We talk about Paula answering phones 24 7, Connie replacing days of marketing work with hours, and why business owners who ignore this shift are living in denial. Marketing drives revenue. Efficiency protects profit. And the gap in the middle is where great businesses are built.
If you are fully booked but still stuck, if you want freedom without adding more staff, and if you want to double revenue before you ever think about multiplying your team, this episode is for you. This one is a wake up call. Less noise. More action. More profit.

In this episode of Paul Meets, I sit down with Jennifer Penrose (https://penrosept.com), a clinic owner from Washington State who built a nearly $2 million physical therapy practice and no longer treats patients herself.Jennifer’s journey did not start with a perfect plan or a trendy business model. She opened her clinic in 2007, spent years stuck in an insurance based grind, and hit a turning point when a competitor opened directly across the street. That moment forced her to rethink everything about growth, marketing, leadership, and what it really means to build a business that lasts. In this conversation, Jennifer shares what actually changed the trajectory of her clinic and how she went from surviving to scaling with confidence. This episode is a must watch for clinic owners who feel comfortable but uneasy, successful but restless, or profitable yet capped by outdated systems. If you want to build a practice that supports your life instead of consuming it, Jennifer’s story will challenge how you think about growth, leadership, and what is actually possible.Like, subscribe, and share this episode with a clinic owner who needs to hear it.#paulgough #thepaulgoughshow #physio #physicaltherapist #physiotherapy #pt #physicaltherapy #privatepractice #businessgrowth #marketing #marketingtips #digitalmarketing #growth #success #mindset #business #sales------------------------In this episode, I’m bringing you something special. PT Reddit Files dives into the shadowy underworld of Reddit, where anonymous PTs and clinic owners vent, rant, and project their frustrations about business without ever confronting the real problem: a refusal to learn the rules of business ownership.
In Case File Two, we dissect a Reddit thread titled “Surviving the Slow Months.” What starts as a familiar complaint about insurance cuts, payroll stress, and “unfair” realities of private practice quickly reveals something deeper: resentment toward business itself and an attachment to how things should be instead of how they actually are.
In this episode, I break down why slow months don’t kill clinics, poor planning does. Why being a great clinician caps you at $20K a month if you never develop business skills. And why blaming Medicare, staff, or patients is a guaranteed way to stay stuck. We contrast the “should world” mindset with the “is world” reality and explain why the clinics that thrive accept the rules of the game and build smarter models around them.
If you’ve ever felt frustrated, burned out, or tempted to believe the profession is broken, this episode will challenge how you think about business, ownership, and what’s actually possible when you stop fighting reality and start designing around it.

In this episode I’m breaking down the difference between dreams and ambitions… why one keeps you stuck and the other gets you paid. I get into what every clinic owner is really chasing – security – and why you’ll never find it in money, marriage, muscles, or more followers. You only get security when you learn how to feel secure before any of those things arrive.
Turn this one up nice and loud because we’re also diving into the two traits that separate the clinic owners who go to the moon from the ones who stay stuck on the launch pad. Accountability and humility. The willingness to ask for help. To admit you don’t know everything. To make the wrong hire, dust yourself off, and go again.
If you’ve ever wondered how some clinic owners seem to rocket into orbit while others grind for years, this episode tells you exactly why.
Hit play, get fired up, and let this one put some heat under your ambition.

In this episode of Paul Meets, I sit down with Keith Roed from South Dakota.
Keith and I first met in 2018. Six weeks before he left his job, he called me and told me he was done. No backup plan. No safety net. Just a decision that his values no longer aligned with where he was working.
At the time, more than one person told him it wouldn’t work.
Seven years later, Keith owns a multi location seven figure physical therapy practice, is completely out of the treatment room, and is now building a second business consulting clinics across the US in regenerative therapies.
In this conversation, we go deep into Keith’s journey.
Why values mattered more than money.
What pushed him to walk away from security.
How he built a team he trusts.
Why small towns are not an excuse for small thinking.
And what clinic owners need to do right now to avoid falling behind as healthcare changes.
We also talk about regenerative medicine, cash services, team buy in, leadership, mindset, and why taking care of people has always been the real growth strategy.
This episode is for clinic owners who know something isn’t right anymore, but haven’t yet made the decision to change it.
Enjoy.

In this special episode of the show, I’m bringing you straight into the room with me for one of the most important conversations we’ve had all year. I just spent two full days here in my Orlando office with business owners who flew in from all over the world to map out their strategic plan for 2026, and what you’re about to hear is real, unfiltered footage taken directly from that event.
Inside this episode, I break down where the physical therapy industry is truly heading by 2030, why digital first and content driven clinics will win, and how buying behavior is already shifting under your feet. I talk about staff shortages, the future of admin work, and what it means to build a personality based business that is rooted in community, experience, and your unique magic.
You’ll hear me challenge some deeply held beliefs about hiring, culture, pricing, and growth. You’ll hear the truth about why Google budgets are failing, why your team must be willing to “sell the wizard,” and why getting to 750 visits a month profitably is the only model that will survive the next decade. I even dig into the cultural differences between the US and UK and how they impact the ambition and mindset of clinic owners everywhere.
Wherever you’re listening from, turn the volume up and enjoy this behind the scenes experience. My goal, as always, is to help you grow a more successful and profitable physical therapy business. Let’s get into it.

In this episode, I go off on one of the most ridiculous things I’ve ever heard in my life: school photo retake day. What started as an innocent conversation with Natalie turned into a full scale rant about how parents are unknowingly programming insecurity, perfectionism, and lifelong anxiety into their kids… all because a picture “wasn’t good enough.”
I walk you through the horror scene of six and seven year olds being marched out of class for the “ugly kids retake line,” the message it sends, and why it’s no wonder those same kids grow into stressed, approval-seeking adults who never feel enough. I even share a few stories of my own son Tobias and the perfectly imperfect school photos that capture exactly who he is, chaos and all.
This is my rant about identity, confidence, and rejecting the obsession with looking perfect whether you’re five, fifteen, or forty five. A picture is meant to capture who you are in that moment and if someone doesn’t like it, the answer is simple… fuck em.
Turn this one up loud. This might be the most important message I’ve shared yet.

In this episode of Paul Meets, I sit down with one of the most impressive new clinic owners in the Planet Paul community, Ricky Morgan (https://morgainphysicaltherapy.com).
Ricky started his practice just over a year ago and has already built a fully booked, three hundred thousand dollar cash clinic. He flew in from North Dakota to Orlando and we spent the day together going deep into his story, his mindset, and his plans to grow to one point five million and complete freedom from patient care in the next few years.
We talk about what it was really like for him to walk away from a safe university job, the influence of his dad and father in law as business role models, the health and life events that reshaped his view of time, and the role his wife plays in giving him the tough love he sometimes needs to take action.
We also get into hiring, leadership, the changing state of the PT profession, and the difference between staying stuck in the system and building a business that gives you true autonomy. If you are at the start of your journey or stuck somewhere on the path to freedom, I think you will see a lot of yourself in Ricky’s story.

In this episode, I’m bringing you something very special. I’ve just spent two full days here in my Orlando office with a group of business owners who flew in from all over the world for our Strategic Annual Planning event. The conversations were big, the insights were powerful, and the next five years of private practice are going to look nothing like the last five.
You’re about to hear real footage from inside that room.
I walk you through what’s changing in our industry, why 2030 is closer than you think, and what today’s AI fueled patient is already expecting from your clinic. We get into the gap between what patients want and what most clinics deliver, how to think about the future of marketing when Google is no longer the primary search engine, and why old school customer service is about to become your most valuable differentiator.
Wherever you’re listening from, turn this up nice and loud. Let me help you build a profitable, modern clinic that stands out in a world that’s moving faster than ever.

In this episode I go off on one of the biggest lies clinic owners tell themselves: I can do it better. Maybe you can. But at what cost? Two weeks of your life for a 15 percent improvement that no one but you will ever notice?
I dive into the trap of perfectionism, why it keeps so many of you stuck, and how embracing the G Spot (the good enoughspot) is the only way to create momentum. I’ll show you how AI like Connie can hand you back nine days of your life, why the pursuit of perfect is a prison, and how business owners who win are the ones who get things out the door fast.
If you’ve ever rewritten the same email six times, fixed spelling mistakes no patient cares about, or convinced yourself that perfection was the path to progress… this rant will shake you awake. Turn it up. Let it punch you in the gut. And start finding the G Spot in your business before another month disappears.

In this episode of Paul Meets, I sit down with the incredible Ellen Tomsic from Durango, Colorado — a clinician of 35 years who went from nearly losing her insurance-based clinic… to building one of the strongest cash pay, wellness-driven practices in the country.
Ellen’s journey is one of the biggest transformations I’ve ever seen.
She went 16 years without tracking a single KPI or cash flow number, was drowning under collapsing insurance reimbursements, and couldn’t keep a second therapist full because everyone only wanted to see her.
Then Covid hit.
And that’s when everything changed.
You’ll hear how Ellen:
• Discovered cash flow forecasting for the first time and completely rebuilt her business model
• Dropped insurance contracts strategically until she reached a 50 percent cash pay model
• Started hosting workshops that went from “no one will show up” to overflowing rooms
• Added wellness, concierge memberships, Pilates, massage, stretch therapy, and regenerative medicine
• Transitioned out of the 9 to 5 treatment grind and built a team that patients trust
• Built a true lifestyle business that lets her ski, travel internationally, raft, and enjoy the life she designed
This is one of the most important conversations you’ll hear if you’re trying to escape the trap of low reimbursement, burnout, and being the only person patients will see.
If you’re a clinic owner who knows there has to be a better way, this episode will show you exactly what’s possible when you choose to adapt instead of suffer.

PT Reddit Files dives into the shadowy underworld of Reddit, where anonymous PTs confidently share their loudest opinions without ever having run a clinic, signed payroll, or made a single marketing decision.
In Case File One, we explore a post from a young PT who believes selling is beneath them. From there we unpack the real issues: insecurity, inexperience, resentment, and the belief that business responsibilities should somehow not apply to clinicians.
This episode reveals what these attitudes really mean for clinic owners and how they show up inside your business long before you notice them.

In this episode, I’m calling out something that has completely vanished from most clinics… basic fucking manners. The stuff that actually builds a business. The stuff everyone else seems too busy, too stressed, or too self-absorbed to remember.
I talk about why I used to make the mailman a coffee every morning, why he delivered my checks while everyone else’s went missing, and how that simple gesture turned into years of free word of mouth. I’ll show you how I learned to greet people properly, how we remembered every patient’s name, and why asking “Are you here for an appointment?” makes you look clueless.
I’m challenging you to make 2026 the year you become known as the clinic with the best customer service in town. Not the clinic with the fanciest equipment. Not the clinic with the best letters after your name. The clinic that actually gives a shit about people.
Because most of what passes as customer service today is embarrassing. There’s no empathy, no eye contact, no warmth, no memory of who the hell walked through the door last week. And I’m tired of watching clinic owners pretend it’s fine.
If you want to grow, start here.
Start with the small things.
Start with fucking manners.
Turn it up and enjoy the rant. It might just be the reset your clinic needs.

In this powerful episode of Paul Meets, I sit down with Joni Redlich (https://kidpt.com/) to unpack how she went from a tiny “dungeon” room and copying what other clinics did… to a thriving, values-driven pediatric practice with OT (and now speech), a 3,000 sq ft space, and a team that delivers deeply personal, family-centered care.
You’ll hear how Joni:
- Broke the “no one will pay cash” belief and doubled revenue after Accelerator
- Built a bridge for parents beyond “Do you take my insurance?” to real, empowered decisions
- Shifted her message from credentials to what parents actually care about (crawl, walk, thrive)
- Scaled compassion without losing the magic: values, standards, and staff mindset
- Faced the hard stuff—raising prices, letting staff go, leading boldly
- Added OT & speech the smart way to support kids over seasons, not sessions
- Moved from “I do it all” to leaders and buffers so she can focus on vision
