近年來,越來越多人擔心胰臟癌是沒救的絕症, 然而隨著科技進步,胰臟癌的早期發現正逐步成為可能。
11月16日被定為世界胰臟癌日(World Pancreatic Cancer Day),這一天旨在全球喚醒社會對胰臟癌的關注與預防意識,推動早期篩檢和激勵研究努力。胰臟癌因其沉默且迅速進展,被譽為極具挑戰性的癌症之一,是全球癌症死亡排名第三的主要原因。世界胰臟癌日用紫色作為象徵色彩,全球多處建築物及地標在這一天以紫色燈光點亮,並透過社群媒體推廣#WPCAD及#PancreaticCancerAwareness,以統一聲音提高公眾意識。
在這一天,除了教育與宣導,也強調患者與家屬的心理與醫療支持,凝聚全球對胰臟癌病患照護與研究的力量,共同為提升早期發現率與改善預後而努力。
抗老醫學不只是關注外表,更在於細胞與代謝的健康。最新的醫學研究讓我們看到,提早照顧胰臟並調節老化過程,是活得更久、更自在的關鍵。
🔬
血液檢測與自動化程序的結合正在改變癌症早期發現的遊戲規則。《Nature Medicine》與《Nature Cancer》發表的最新研究表明,多重體學技術(multi-omics)結合機器學習演算法能顯著提升胰臟癌的早期偵測能力。其中,研究人員分析超過 5,000 份血液樣本,利用多重體學整合蛋白質組學、脂質組學與臨床數據,在胰臟癌臨床症狀出現之前,便能以接近 93%~97% 的準確度成功識別出早期患者。特別值得注意的是,當多重體學標誌物與傳統腫瘤標誌物 CA19-9 結合時,早期胰臟癌的檢測準確率更能達到 97%。
在這些生物標誌物中,血漿蛋白質被證實是最強的單一預測因子,其預測準確度達 75% 以上,遠勝過傳統的 CA19-9(靈敏度不足,特異性也受限)。這些生物標誌物與「身體老化」的路徑——如慢性發炎、代謝壓力、線粒體功能障礙、以及氧化壓力——高度重疊。換句話說,我們的身體其實正在用老化的節奏,悄悄預告胰臟可能的危機。
液態活檢技術也在快速進展中。微核糖核酸(microRNA)檢測已經展示了令人鼓舞的成果:一項多中心研究發現,三種特定微RNA組成的面板能以 94.4% 的準確度預測早期胰臟癌,且能有效區分胰臟炎與胰臟癌(準確度達 91.5%)。更重要的是,當微RNA簽名與 CA19-9 結合檢測時,早期胰臟癌的檢測準確度達到 91%~97%。
此外,循環腫瘤 DNA(ctDNA)與蛋白質標誌物的組合也顯示出強大的檢測潛力:研究表明,KRAS 基因突變與四種蛋白質生物標誌物的結合能檢測到近 64% 的可切除胰臟癌,同時保持 99.5% 的特異性。
💡 什麼是「沉默殺手」?
胰臟癌早期症狀極不明顯,當黃疸、食慾不振、體重減輕等症狀出現時,通常已是晚期。這正是因為慢性發炎與氧化壓力累積,讓胰臟細胞修復機制提前衰退,導致了細胞層面的老化與失衡。
新研究進一步揭示:DNA甲基化年齡加速(epigenetic age acceleration)與胰臟癌風險密切相關。換句話說,某些人在分子層面上衰老的速度超過其實際年齡,這正是胰臟癌發生的重要風險信號。該研究分析了 393 例胰臟癌患者與 431 例對照者的血液樣本,發現 DNA 甲基化年齡加速與胰臟癌風險呈現複雜的非線性劑量反應關係,並可能作為實用的胰臟癌風險預測工具。
mRNA胰臟癌疫苗:抗癌新希望
近年mRNA癌症疫苗技術取得突破,2024至2025年間多項臨床試驗中,個人化mRNA疫苗結合免疫檢查點抑制劑(checkpoint inhibitors)在胰臟癌患者中展現長期免疫反應及延緩癌症復發的潛力。Memorial Sloan Kettering癌症中心與BioNTech合作研發的個體化mRNA疫苗,面向胰管腺癌(PDAC),能針對患者獨特的腫瘤新抗原誘發強烈CD4+及CD8+ T細胞反應,部分患者接受疫苗治療後,免疫效果可持續近4年,三年復發風險顯著降低。
國際上也正進行mRNA疫苗與PD-1免疫治療組合的臨床試驗,正評估其在術後輔助治療中的安全性與療效,希望通過激活腫瘤特異性的T細胞,將「免疫冷」腫瘤轉化為免疫活躍狀態,提升患者的生存期與生活質量。
mRNA疫苗具有快速定製、精準針對腫瘤特異抗原的優勢,可能逐步成為胰臟癌及其他傳統免疫治療抵抗癌症的新型有力武器。​
🧬 深入醫學機轉:為什麼會發生?
慢性低度發炎是胰臟癌發展的主要驅動力。慢性發炎會持續激活促癌訊號通路,其中 NF-κB 和 STAT3 是兩個關鍵的轉錄因子。這兩個訊號轉導因子形成複雜的調節網路:NF-κB 能上調 IL-6 的表現,而 IL-6 進而激活 STAT3,兩者交叉對話形成正反饋循環,不斷推動癌細胞的增殖與侵襲。研究證實,IKK/NF-κB 和 STAT3 通路的聯合抑制可能代表對抗胰臟癌的有前景的治療策略。
胰臟癌細胞經常伴隨糖代謝異常(如高糖解作用)與線粒體功能障礙,導致胰島素與胰高血糖素失衡。肌肉是全身最大的葡萄糖吸收庫,負責調節 80% 以上的進食後血糖攝取,養好肌肉就是養好胰臟。
胰臟癌細胞會發生代謝重編程:傾向透過糖酵解快速產生 ATP(瓦爾堡效應)。代謝轉移導致乳酸積累、細胞外酸化,並大幅增加活性氧自由基(ROS)。線粒體基因的突變(如 MT-ND4)會破壞電子傳遞鏈完整性,導致更多 ROS 產生與代謝混亂。癌細胞上調轉錄因子 NRF2,協調抗氧化與代謝適應蛋白,維持氧化還原平衡,助癌細胞存活。
每位患者的腫瘤基因譜不同。超過 90% 患者攜帶 KRAS 熱點突變,野生型則富含其他 MAPK 通路改變。 KRAS 等位基因不平衡與預後相關,尤以全基因組倍增患者最顯著。全基因組測序已能於中位 35 天報告結果,助精準篩查與治療。
💡 如何「預防得更早」:呂醫師的抗老處方
1️⃣基礎生活干預
減少精製碳水,增加蔬菜與蛋白質,減輕胰臟負擔。
每天至少 15 分鐘肌肉刺激運動(如快走、深蹲)。運動激活 PGC-1α/TFAM 促進線粒體生物生成,改善胰島素敏感性與代謝功能。
補充維生素 D、Omega-3,調節胰島素敏感性、減少慢性發炎。維生素 D 促進胰島素分泌,Omega-3 改善內皮功能。
2️⃣進階精準醫療
觀察消化變慢、腹脹、體重下降等「低聲訴說」。
高風險人群(家族史、糖尿病患者)搭配液態活檢(微RNA、ctDNA 甲基化分析、新型血清標誌物 TFF2)提高篩檢敏感度。
家族史風險量化,配合新發型糖尿病 ENDPAC 模型,識別監測對象。
動態追蹤代謝數據及分子檢測,調整生活策略。

回溯到 1874年的今天(11月15日),丹麥生理學家奧古斯特·克羅(August Krogh)誕生了。這位 1920年諾貝爾獎得主 🏆,留給後世一個改變醫學觀點的重大發現——「微血管的開關機制」。

💡 克羅告訴我們一個簡單卻震撼的真理: 當身體安靜時,大部分的微血管是「關閉」的 🔒;唯有在 運動 🏃 時,這些微細的通道才會大量開啟,讓氧氣真正滋養到組織深處。

恰逢克羅誕辰與 世界COPD日 重疊的今天,這句話更顯得震耳欲聾: 🗝️「運動,是打開身體氧氣通道的唯一鑰匙。」

今年發表在權威期刊 《The Lancet Respiratory Medicine》 📑 的一項大型跨國研究,分析了近八萬名數據後發現:

📊 「即使藥物治療再先進,若忽視『骨骼肌功能退化』與『身體活動量下降』,病情仍會失控。」

研究特別指出,當 「肺功能下降 📉」 遇上 「肌肉流失(肌少症)🥀」 時,風險幾乎翻倍!這再次印證了克羅百年前的智慧—— 🚫 不動 ➡️ 微血管關閉 ➡️ 肌肉萎縮 ➡️ 人就老得快!


🏞️ 被縮小的生活圈:從象山到客廳的距離

這讓我想起門診一位愛漂亮的阿姨 💃。她以前最愛跟姊妹淘爬象山,拍照打卡總是笑得最燦爛 📸。但隨著 COPD 症狀加劇,她開始怕喘、怕跟不上隊伍,漸漸地連樓梯都不敢爬 🪜。

經過身體組成分析,我們發現她已是嚴重的 「肌少症」 📉。 因為怕喘而不動,因為不動肌肉更萎縮,最後連走到客廳都覺得累... 🛋️ 這不只是器官生病,而是整個生活品質與自信心的崩塌 💔。


🌏 特別提醒身處亞洲的我們: 許多女性即使不抽菸 🚭,卻因長期暴露在 PM2.5 🌫️ 或 廚房油煙 🍳 下,成為「非吸菸型COPD」的高危險群。 🛡️ 對抗環境老化,我們需要的防護不只是口罩,還有強健的肌肉!


💊 呂醫師的處方:把「氧氣」練回來

最新的醫學實證給了我們極大的希望 ✨:規律的 「呼吸肌訓練」 加上 「下肢阻力運動」,能大幅提升生活品質!

我常跟病人說,健康的肌肉就是身體的「氧氣幫浦」⛽。作為抗老醫師,我建議您從今天開始執行這四點 「🔄 逆齡呼吸計畫」:

🧘‍♀️ 啟動深層呼吸 每天 3分鐘 練習「腹式呼吸」,讓橫膈膜重新有力,促進肺部充分擴張。

🔛 打開微血管開關 每週至少 3次,每次 20分鐘 的低強度步行 🚶‍♀️ 或深蹲。記得克羅說的,動起來,氧氣才進得去!

🥗 營養就是地基 補充足夠的 蛋白質 🍗(修補肌肉)、維生素D ☀️ 與 Omega-3 🐟(抗發炎)。沒有原料,身體無法重建肌肉。

🍃 淨化環境 遠離煙害 🚭,煮飯務必開啟抽油煙機,給肺一個乾淨的家 🏠。
